Certainly in England, player exchange deals are very rare. They're almost always done as two separate transactions in case one of them falls through. The most recent cases of effective exchanges go back to this time last year when clubs were trying to fix their PSR holes by selling players to each other, and if memory serves they were all done separately, as much to try to establish the values needed on each leg of the transaction. I also think in some of those cases that more than 2 clubs were involved in the chains.
Exchanges are just messy. Transfer deals can be complex in terms of instalments, incentives etc. and it's just easier to do one agreement per deal - less scope to cock it up. It also means you don't end up with agents seeing the paperwork for players who aren't their clients because it's unlikely the same agent represents both players.
Certainly when I was at the club there were a couple of exchanges that were talked about, but we never did them. The closest one I remember was Robertson from Hull with Kevin Stewart going the other way - two separate deals.
We're all just going to have to put up with this, wirtz an' all.
